Regarding Jim Stasheff's query: I regret that I don't know any extant good exposition on any of the topics (not surpising on 3) since that's out of my area). However, David Kazhdan and Serge Gelfand are apparently working on a book on tensor categories (i.e. monoidal categories, though possibly with some linearity or abelianness required), which may fit the bill on 1) and 2).
